Riassunto Questo lavoro è estratto da un atlante provinciale che valuta la distribuzione territoriale di ricoveri e di prestazioni ambulatoriali. Sono qui commentate alcune cartine di distribuzione dei consumi nei residenti per Distretto confrontati coi consumi provinciali. Ne risulta una variabilità spesso non dovuta a motivi epidemiologici: vi sono prestazioni più utilizzate dove l'offerta è maggiore e probabilmente vi sono diverse attitudini diagnostiche e di organizzazione nel trattamento.
È opportuno verificare se gli eccessi di consumi si traducano in reali vantaggi di salute.

Summary This study is based on a provincial Atlas assessing territorial differences in hospital admissions and out-patient services. The authors have analysed consumer distribution per District as compared to overall provincial consumption. Results show some geographical inequities that are not always associated to epidemiological reasons. As a matter of fact, some services happen to be more crowded than others, which obviously entails the need for more adequate offers and different diagnostic behaviours as regards treatments. It would therefore be advisable if high consumption levels were assessed to be effectively useful in terms of health production.

Le recours aux services sanitaires dans l'Unité Sanitaire Provinciale de Varèse

Résumé Cette étude repose sur un atlas provincial qui apprécie la distribution territoriale à partir des hospitalisations et des services de dispensaire. Les auteurs ont analysé le taux de consommation sanitaire par District, le comparant à la consommation générale dans la Province. Il en résulte une variabilité géographique qui ne tient pas toujours à des raisons épidémiologiques. Il existe des services qui sont plus utilisés par rapport à d'autres, ce qui implique une offre plus importante et des démarches diagnostiques et organisationnelles différentes vis à vis du traitement.
Ainsi, il importe de vérifier si ces niveaux de consommation sanitaire sont effectivement utiles en termes de production de santé.

Zusammenfassung Als Grundlage der Arbeit dient ein Atlas der Provinz in dem die territoriale Verteilung der Krankenhauseinlieferungen und der ambulanten Behandlungen ausgewertet sind. Es werden Verteilungskarten diskutiert, in denen die medizinische Inanspruchnahme durch die Einwohner in den einzelnen Bezirken mit derjenigen der Provinz insgesamt verglichen wird. Die festgestellte Variabilität hat oft keine epidemiologischen Ursachen: Medizinische Dienstleistungen werden anscheinend vorwiegend da in Anspruch genommen, wo ein größeres Angebot vorliegt;
aber wahrscheinlich werden auch bestimmte Usancen in Diagnostik und Therapie bevorzugt. Es wäre noch zu überprüfen, ob die Häufung medizinischer Inanspruchnahmen auch tatsächliche Vorteile für die Gesundheit mit sich bringt.
